Sciatic Pain in Athletes: What You Need to Know

Don’t let nerve pain keep you sidelined. Here’s what causes sciatic pain in active individuals — and how to treat it effectively.

What Is Sciatic Pain?

Sciatic pain refers to discomfort, tingling, numbness, or a burning sensation that travels along the path of the sciatic nerve, which runs from the lower back, through the hips and glutes, and down the back of each leg.

For athletes, sciatic symptoms can show up during sprinting, lifting, deep squats, or prolonged periods of sitting or standing — limiting performance and often leading to compensations elsewhere in the body.

Common Causes in Athletic Populations

While disc herniations and lumbar spine conditions are well-known causes, many athletes experience sciatic pain due to soft tissue restrictions of the deep hip rotators — including the piriformis, gemelli, and obturator internus muscles.

These small but important muscles stabilize the hip and pelvis during high-demand movements. When they become tight, overused, or imbalanced (often from running, squatting, or sitting), they can irritate the sciatic nerve as it passes beneath or through the area.

Nerve Sheath Irritation

In addition to mechanical compression, sciatic pain can result from irritation of the nerve sheath — the protective layer surrounding the nerve. This sheath can become inflamed or hypersensitive due to poor mobility, inflammation, or overuse, especially in the posterior hip and leg. This leads to symptoms resembling classic sciatica, even without a true disc-related issue.

Effective Treatment Strategies for Sciatic Pain in Athletes

At St Pete Physical Therapy, we specialize in performance-based rehab that targets the root cause of your sciatic pain. Here’s how we help athletes recover quickly and safely:

🔹 Dry Needling

An excellent tool to improve blood flow, decrease tone in overactive deep hip rotators, and reduce sensitivity of the neural tissue. Especially effective for calming down irritable nerve pathways.

🔹 Cupping Therapy

Used to release fascial restrictions, improve circulation, and decompress the tissues surrounding the sciatic nerve, creating more space and mobility in the posterior hip.

🔹 Targeted Manual Therapy

Hands-on techniques to mobilize restricted joints and soft tissue, especially in the hips, lumbar spine, and glutes, to reduce compression and restore normal motion.

🔹 Neurodynamic Mobilizations

Specialized movements to restore normal gliding and mobility of the sciatic nerve as it travels down the leg, reducing mechanosensitivity and restoring function.

🔹 Strength & Mobility Programming

Customized exercises to address strength imbalances and improve core, hip, and lumbar stability so you can get back to performing at your highest level — pain-free.

Don’t Just Rest — Rehab the Right Way

Rest alone won’t fix sciatic issues in active individuals. The key is to identify and treat the true source of irritation, restore normal nerve movement, and rebuild strength where it matters most.

Whether you’re dealing with a recent flare-up or a nagging issue that’s been affecting your training for months, we’re here to help.
